Valid SCAC (National Motor Freight 703-838-1810) preferred
Completed W-9
Satisfactory Safety Rating (www.safersys.org)
Insurance requirements, including $1 million in Auto Liability, and General Liability of $1 million per occurrence, $1 million Aggregate, $100,000 Cargo Liability; PLS has an A.M. Best Rating of at least A (www.ambest.com) required for some customers
